Community Health Service Inc. (CHSI) relies on dedicated providers like you to serve the Dental needs of our patients including farm workers and their family members. You are among a network of providers in Minnesota and North Dakota whose commitment has resulted in a significant number of farm workers being able to access dental care which in turn has led to the improvement of their overall dental health.
CHSI begins it’s new project year on February 1 every year. Unfortunately CHSI does not have the financial capabilities to pay for all services that are rendered by providers to persons registered within our clinics. The patient, parent or guardian is responsible for paying the balance owed once CHSI has paid the maximum allowed amount of $200 per patient per year for Emergency and Preventive Dental Care Service. CHSI’s fiscal year is February 1 through January 31.
Patients need prior approval from the health center or our central office in order to benefit from this service. The patient will present with a voucher at the time of the appointment before dental care is provided. Should a patient fail to attend their appointment, the dental provider may bill the patient for the cost of the no show.
