Rhonda Eastlund
Chief Executive Officer
Rhonda Eastlund is an experienced healthcare leader who's served as CHSI's Chief Executive Officer since June 2024. Rhonda started her career as an AmeriCorps member in Minneapolis before moving into community health and development. She's served in leadership roles for several community health centers, including People's Center Health Services and the Brian Cole Community Center in Minneapolis, the Community Health Association of Spokane and Columbia Valley Community Health Center in Washington, and Chicago Family Health Center. Prior to joining CHSI, Rhonda served as the CEO of Open Door Health Center in Mankato where she developed their dental, chiropractic, and school-based operations. She earned her B.A. at Augsburg College, and she enjoys biking with her husband, gardening, and spending as much time as possible with her three grandchildren.
Rhonda is proud to be part of an organization that serves the full diversity of people in outstate Minnesota and North Dakota. She believes the access we provide to behavioral, oral, and physical health services, as well as victim intervention and advocacy, strengthens our communities.

Dr. Cynthia Woods
Chief Medical Officer
Dr. Cynthia Woods is an internal medicine physician and serves as CHSI's Chief Medical Officer. Dr. Woods earned her doctorate in osteopathy from Philadelphia College of Osteopathic Medicine and a B.S. in biology from Baylor University. She completed her residency at Penn State Hershey Medical Center, and she is certified by the American Board of Internal Medicine. Dr. Woods previously served in leadership at Open Cities Health Center and as a staff physician at Crescent Community Health Center. In addition to her work at CHSI, she currently serves as the Senior Vice President of Medical Care at Minnesota Community Care.
Dr. Woods is passionate about addressing social and structural drivers of health, health inequity, and historical trauma. She is driven to provide exceptional care for CHSI patients.

Lucia Sanchez
Intervention Director
Victim Advocacy Program
Lucia Sanchez joined CHSI in May of 2000 as a Bilingual Intervention Advocate, providing crisis assistance to survivors of domestic and sexual violence in our community. Since 2022, she has served as the Intervention Director of our Victim Advocacy Program. Lucia is a member of the Minnesota Coalition Against Sexual Assault and Violence Free Minnesota. She is also an ordained minister who has volunteered as an interim pastor and Sunday School teacher at her local church in Moorhead. She spends her free time with her children and grandchildren, and she is passionate about thrifting and flea markets!
Lucia enjoys working with a passionate and dedicated team working to provide quality care, promote safety, and eliminate violence in our communities.

La Tonya Clark
Executive Assistant

La Tonya Clark is CHSI's Administrative Assistant and Compliance Officer. She earned an A.A.S. in medical office administration from Brown College in Brooklyn Center, Minnesota, and also completed the nursing assistant program at Minneapolis Community College. Her years as a healthcare administrator and union leader provide valuable perspective to CHSI's operations and equity initiatives. In her free time, La Tonya enjoys traveling, shopping, and spending time with her friends and family.

Management Team
Avantika Singh is CHSI's Accountant. She earned her B.S. in international studies from Minnesota State University Moorhead in 2013 and worked in the hospitality industry for more than 10 years before joining Community Health Service Inc. As part of the financial team, Avantika works to maintain CHSI's fiscal health across all five of our sites. In her free time, Avantika enjoys art, music, reading, swimming, and spending time with her family.

Claudia Becerra is the Site Manager at CHSI's Willmar clinic. She brings more than 17 years of management and 24 years of customer service experience to our operations, and she also maintains several insurance licenses from her former professional career. In her free time, Claudia enjoys cooking, catering, and preparing income tax returns. She lives in Willmar with her husband of 25 years, and they have two daughters.

Jennifer Holtz is the Site Manager at CHSI's Moorhead clinic. Jennifer earned a B.A. in psychology from the University of North Dakota and an M.S. in public human service and health administration from Minnesota State University Moorhead. She has worked over 20 years in mental health care and has found that her greatest passions are in connecting people to the information and resources they need. In her free time, Jennifer enjoys dancing, sports, and spending time outside with her dog Maggie. She also serves on the City of Fargo's Planning Commission.
Jennifer values social justice and health equity, and she's glad that she works with so many colleagues who share those values.

Leoconstance Sannor is the Human Resources Manager at CHSI. She drives the organization's talent acquisition and HR operations, and she helps foster a positive workplace culture. Leo earned her B.S. in human resources management from Metropolitan State University and an M.H.A. from Walden University. In her free time, she enjoys cooking and spending time with her family.

Mark Klampe is the Site Manager at CHSI's Rochester clinic. He earned a bachelor's degree in sociology and criminal justice from the University of Wisconsin - Madison and brings a wealth of management experience to our operations. Mark and his wife Jasmine have started three businesses in Rochester, including a promotional products shop, a restaurant, and a comedy club. Mark enjoys road trips, movies, and spending time with his wife and two children.
While Mark thinks CHSI's mission is fantastic, his favorite part about clinical operations is his coworkers — who bring kindness, fun, and diversity to work every day.

Kate Suda is a Physician Assistant who has provided care from CHSI's Grafton clinic since 2023. A native of the Red River Valley, Kate earned her bachelor's in psychology from NDSU and her M.S. in physician assistant studies from the University of North Dakota. Kate is licensed in Minnesota and North Dakota, and she is certified through the National Commission on Certification of Physician Assistants. Kate lives with her husband Derrick and their two sons in Grafton. In her free time, she enjoys cooking, baking, and gardening.

Lucy Brink is a Nurse Practitioner who practices from our Willmar clinic. Lucy earned her bachelor's in nursing from the College of St. Benedict and her M.S. from St. Catherine University. She has worked as a nurse practitioner for 10 years, including six years at a diabetes center, and she is board certified as an adult/gerontological NP and a diabetes care and education specialist. Lucy lives in Willmar with her husband Dan and their two children. She enjoys spending time outdoors, traveling, cooking, and learning.
Lucy is passionate about chronic disease management and motivated to make an impact in her community.

Marie Anderson is a Nurse Practitioner in our Grafton clinic. She joined CHSI in 2023 after serving many years in family practice. Marie earned her bachelor's in nursing from the University of Mary and her M.S.N. from the University of North Dakota. She is licensed in both North Dakota and Minnesota and certified through the American Nurses Credentialing Center. In her free time, Marie enjoys gardening, biking, hiking, and reading. She lives in Drayton with her husband, and together they have three children and 6 grandchildren.

Richard Mokua is a Nurse Practitioner in our Rochester clinic. He joined CHSI in 2022 after specializing many years in nutrition and internal medicine. Richard was born and raised in southwestern Kenya and studied food science and education before migrating to the United States, where he earned a B.S. in nursing from UW-Eau Claire, an M.S. in nutrition from UW-Stout, an M.S. in nursing from Winona State University, and a doctorate in nursing practice from Chamberlain University. Prior to joining CHSI, Richard practiced internal medicine at Mayo Clinic, and he's served in the U.S. Army Reserves since 2009. Richard spends his free time with his family and enjoys jogging and DJing.

William "Patricio" Aleman is a Physician Assistant providing care in our Rochester clinic. Patricio earned his M.D. from Central University of Ecuador, a master's in physician assistant studies from the University of California - Davis, and a master's in adult education from the University of Minnesota - Rochester. He brings decades of experience in primary care at Mayo Clinic and has been certified by the National Commission on Certification of Physician Assistants since 1990. Patricio lives with his wife in the Rochester area and enjoys reading, traveling, and spending time with his children and grandchildren.

Alma Gomez is a Spanish-language Interpreter who works from our Rochester clinic. She first worked as an executive assistant in Guadalajara, Mexico, and then earned her GED and CNA after she migrated to the United States. She holds several certifications, including one in MNSure Navigation. Alma lives in St. Charles, MN, with her husband and one of their four sons. In her free time, she leads several programs with her church, and she enjoys walking, dancing, and singing.

Claudia Cazenave is a Registered Nurse who works in our Rochester clinic. Claudia earned her associate degree in nursing from Rochester Community and Technical College and subsequently passed the NCLEX to become a licensed RN. She also has an associate degree in business administration from the Propam School of Business in Santiago, Chile. Claudia is bilingual, speaking English and Spanish, and she is a certified diabetes care and education specialist. In her free time, she enjoys travel, hiking, kayaking, and painting. She lives in the Rochester area with her husband. They have one child.

Claudia earned her LPN and RN ADN degrees from Rochester Community and Technical College and then her B.S. in nursing from Augsburg University. She carries almost 20 years of experience in healthcare, she is bilingual (Spanish & English), and she is currently enrolled in the family nurse practitioner program at the University of North Dakota. Haley Suarez is a Patient Relations Representative in our Grafton clinic. She brings several years of experience in healthcare and customer service to our operations, having previously worked in security and front desk roles in clinical and family practice settings. Haley lives in the Grafton area with her husband and their husky, Dutchess. She enjoys reading, camping, and playing guitar in her free time.

Hope Barry is a Registered Nurse and our Care & Referral Coordinator based in our Grafton clinic. Hope brings more than 35 years of experience in healthcare, progressing from CNA, CMA, LPN, and RN to her present position. She is licensed in Minnesota and North Dakota, certified as a diabetic educator, and a member of the Care Coordinator Association of Minnesota. Hope lives in rural Larimore with her husband, and in her free time she enjoys reading, being outdoors, and spending time with her friends and family.

Jenifer Barroso is an LPN who works in our Rochester clinic. Jenifer started her healthcare career in high school as a nursing home dietary aide. In the years since, she's advanced her work and education as a CNA, Pediatric Dental Assistant, and currently an LPN studying to become an RN. Jenifer enjoys reading, watching K-dramas, and spending her weekends hiking or with family. She currently lives her fiancé and they're hoping to hike all of the U.S. National Parks.

Katherine Condor Dockter is the Outreach & Enrollment Specialist in our Moorhead clinic. Originally trained as a nutritionist in Perú, Katherine moved to the United States and first worked as a CNA and MA at a nursing home. Finding her passion in public health, she joined CHSI and has since become a certified MNSure Navigator and Community Health Worker. Katherine lives in the Fargo area with her husband and their two sons. She likes to travel, camp, and spend time with her children.

Katie Peterka is a Registered Nurse in our Grafton clinic. Her career in health care started at Altru, where she worked as a CNA while pursuing her LPN degree at Northland Community and Technical College. She passed her RN board exams after joining CHSI. Laysha Carrillo Gonzalez is a Patient Relations Representative in our Rochester clinic. She assists our patients and clients with check-in and registration. Mary Lisa Estrada is the Outreach & Enrollment Specialist in our Rochester clinic. Prior to entering the healthcare field, Mary graduated from St. Cloud Technical College with an associate degree in credit and finance and worked for a variety of financial and social service institutions, including the Social Security Administration, Goodwill, the Olmsted County Child Support Division, and Liberty Tax. Since joining CHSI, Mary earned her certifications as a MNsure Navigator and Community Health Worker. Mary lives in Eyota with her partner of 44 years, and she enjoys camping, hiking, history, and spending time with her family and friends.

Olga Mendoza Negrete is a Certified Medical Assistant who works in our Rochester clinic. She joined CHSI in 2014 as a Patient Relations Representative and was then promoted to Office Manager before completing her training as a CMA. Olga has two sons and lives in the Rochester area. In her free time, she enjoys soccer, volleyball, hiking, and Marvel movies.

As an Outreach & Enrollment Specialist, she works to promote clinical operations and to connect our community members with the care and services they need. Paulina earned an associate degree in liberal arts from Ridgewater College with a focus in communications studies. She is MNSure certified, allowing her to help our patients and clients navigate insurance options. In her free time, Paulina enjoys watching movies, listening to music, and shopping online. Terra Sondreal is a Certified Medical Assistant providing care in our Moorhead clinic. Terra is a nationally certified medical assistant and holds years of experience from a variety of healthcare practices, including internal medicine, ophthalmology, occupational health, and gynecology and obstetrics. Behavioral Health Team
Gracia Do is the Behavioral Health Supervisor and a Licensed Professional Clinical Counselor at CHSI. She earned her bachelor's degree from Minnesota State University and her M.S. from St. Mary's University. Gracia brings almost a decade of experience providing therapy to individuals of all ages, and she is trained in eye movement desensitization and reprocessing (EMDR) therapy, trauma-focused cognitive behavioral therapy, and dialectical behavioral therapy. Her expertise includes the treatment of post-traumatic stress, depression, anxiety, panic attacks, and relationship conflicts. Gracia enjoys travel, interior design, working out, and spending time with family and friends.

Dental Team
Dr. Philip Sallberg serves as the lead dentist and dental director at Community Health Service Inc. He graduated with his D.D.S. from the University of Minnesota and spent almost 40 years in private practice in Ada and Roseau, Minnesota. While practicing in Ada, he also worked with Migrant Health Service Inc (CHSI's predecessor) to provide evening dental service. He now provides comprehensive dental care for CHSI's patients. Dr. Sallberg lives with his wife in the Fargo area and spends his time with their children and grandchildren — or playing pickleball!

Virginia Baker
Intervention Advocate
Moorhead Office
Virginia Baker is an Intervention Advocate based in CHSI's Moorhead office. Virginia grew up on the White Earth Reservation in western Minnesota and moved to Moorhead in 2023. As an advocate in CHSI's Victim Advocacy Program, she helps victims of domestic and sexual violence access counseling, safe and secure housing, and other legal and community resources. In her free time, Virginia enjoys tending to her vast plant collection and hiking in state parks.
Virginia enjoys her work at CHSI because she is able to help community members every day.
