CHSI Dental
Community Health Service Inc. offers a variety of dental services at our Moorhead clinic, including comprehensive exams, cleanings, fluoride treatments, sealants, x-rays, fillings, extractions, and emergency/walk-in care. We also offer services on select dates at our other clinics and through our Mobile Dental Unit.
Community Health Service Inc. is part of the Delta Dental network and accepts most private insurance plans. We also accept Medical Assistance, MinnesotaCare, Medicare Supplement and Advantage plans, and provide service on an income-based sliding fee scale.

Dr. Philip Sallberg
Dental Director
DDS
Dr. Philip Sallberg serves as the dental director at Community Health Service Inc. He graduated with his D.D.S. from the University of Minnesota and spent almost 40 years in private practice in Ada and Roseau. While practicing in Ada, he also worked with Migrant Health Service Inc (CHSI's predecessor) to provide evening dental service.
Dr. Sallberg lives with his wife and spends his time with their children and grandchildren — or playing pickleball.

Jill Lemke
Dental Manager
LDA, RDA, RF
Jill Lemke serves as the dental manager at Community Health Service Inc. She studied dental assisting at Minnesota State Community and Technical College and has worked in dentistry in Fargo-Moorhead for 14 years. She enjoys providing dental service at CHSI because she likes to see her patients smile.
Jill lives with her partner and their three dogs. She enjoys working out and learning guitar (especially songs by The Pixies and Mazzy Star).

Casey Sabo
Dental Hygienist
RDH
Casey Sabo is a Dental Hygienist who provides care in our Moorhead clinic. She studied dental hygiene at Rochester Community and Technical College and she is licensed as a dental hygienist in both Minnesota and North Dakota. Casey lives in Fargo and enjoys golfing and going to the lake in her free time.
Casey's favorite part about working at CHSI is the people.
