Are you passionate about improving your community’s health and building better health care options for your family, friends, and neighbors?

We want your feedback to improve patient experience.

Next PAC Meetings:

Rochester - Tuesday, April 9, 12PM

Moorhead - Thursday, April 11, 12PM

Grafton - TBD

Willmar - TBD

Roles & Responsibilities

CHSI's Patient Advisory Committee meetings are held quarterly over the lunch hour — either online or on location at CHSI's clinics (lunch provided on site). Committee members are asked to provide patient input on clinical matters and help guide CHSI's quality improvement initiatives.

Patient Advisory Committees are formed at each clinical site (Grafton, Moorhead, Rochester, Willmar) and comprise 4-8 non-board patients and two CHSI staff members. One patient member will serve as committee chair and one staff member will serve as committee secretary.

Mission

The Patient Advisory Committee is dedicated to ensuring that Community Health Service Inc. provides high quality and compassionate health care. The positive changes we create through communication and collaboration will foster an environment where patients and family members feel respected and empowered to be partners in their health care.
