Rochester is home to CHSI’s only year-round site in southern Minnesota. Housed in the Heintz Center at Rochester Community and Technical College. CHSI has developed great relationships with Mayo Clinic, Salvation Army, Winona State University, and numerous other local healthcare, education, and community organizations in the Rochester area. The Rochester clinic is staffed with two Physicians and a Nurse Practitioner.
Dr. Low is available Mondays and Thursdays, Dr Rho is available all day Tuesdays and a half day on Wednesdays (8am-12:00pm) and Joan Grotwald FNP-C is available all day Mondays, a half day on Wednesdays (1pm-4:30pm) and all day on Fridays. .

Both Cristy and Mary are Certified MNSURE Navigators and can assist with MNSURE enrollments.
They can also assist patients with finding community resources in the surrounding area, including but not limited to:

- Assist with enrolling into State Programs like Medical Assistance and Minnesota Care, year round
- Scheduling specialty appointments outside of Community Health Service Inc.
- Referrals to other Community Services
