Medication Assistance

As a patient of our health center, you may be eligible to obtain affordable medications through pharmacy assistance programs or at a reduced-cost at specific pharmacies. We will help determine eligibility for participation and assist with the applications.

Insurance

Community Health Service Inc. (CHSI) accepts most types of health insurance, including Medicaid, Medicare and most commercial plans. Please call us to find out if we accept your insurance. We request that patients without insurance meet with a navigator to determine eligibility for public health insurance or other programs that will help pay for your health care services.

Sliding Fee Program

If you do not carry medical or dental insurance or if your insurance does not cover certain services, you may be eligible to enroll in our Sliding Fee Program. Fees are discounted based on household size and income. You will be asked to provide proof of household income, usually the 1040 tax form from the most recent year, a copy of your W-2s, Social Security or pension statements, pay stubs, and any other income received by your household prior to obtaining services. If you do not qualify for the Sliding Fee Program, you will be billed for the full amount.

Billing

A nominal fee will be applied, according to your income and household size, from the sliding fee discount program. CHSI offers a discount if your nominal fee is paid the day of service. Patient billing statements are sent out monthly. For questions about your bill, call Kim at 218-236-6502. To make a payment call your local clinic or pay online.

Other Healthcare Programs

You may be eligible for other state offered programs, such as SAGE (MN) and Women’s Way (ND). These state screening programs are offered through the Minnesota Department of Health and the North Dakota Department of Health for women age 40 and older who qualify based on income guidelines.
