Prenatal and Chronic Disease
While CHSI provides health care to all patients, we offer specific programs for two groups: pregnant women and individuals with chronic diseases such as diabetes, high blood pressure, and depression.
CHSI’s prenatal program offers personalized nursing care, education, and case management focused on the individual needs of each woman. This may include referrals for dental care, WIC, or for other services in the local community. Patients receive culturally sensitive prenatal education and referrals to area medical providers for continued prenatal care.
Perhaps one of the most unique facets of CHSI’s chronic disease program is the annual cluster clinics. These “one-stop shops” allow patients with diabetes and hypertension to visit with several specialty providers all in one evening. Now you won’t have to make multiple visits or take time off work to see the specialized providers you need!
Case Management and Voucher Referrals
CHSI is one of only about 20 voucher programs in the nation. This extensive voucher program links CHSI patients to a far-reaching network of approximately 650 community providers. These providers are spread out all over the state so chances are you’ll have options close to home. Patients can see a doctor or a specialist for as little as $22.50. Vouchers also are available for laboratory tests, x-rays, and dental services.
Pharmacy vouchers bring the cost of most brand-name medications down to $22.50 each per month. CHSI advocates the use of the multitude of generic drug plans available, including its own self-pay program that has garnered national attention. Strong relationships with community pharmacies have let the program grow to more than 150 participating locations.
CHSI’s comprehensive case management services are at the core of its mission of delivering quality health care. Nurses, community health specialists, and bilingual outreach workers can connect you to community resources, assist with applications for social service programs, and facilitate appointments with various agencies.
