Dr. Cynthia Woods
Chief Medical Officer
Dr. Cynthia Woods is an internal medicine physician and serves as CHSI's Chief Medical Officer. Dr. Woods earned her doctorate in osteopathy from Philadelphia College of Osteopathic Medicine and a B.S. in biology from Baylor University. She completed her residency at Penn State Hershey Medical Center, and she is certified by the American Board of Internal Medicine. Dr. Woods previously served in leadership at Open Cities Health Center and as a staff physician at Crescent Community Health Center. In addition to her work at CHSI, she currently serves as the Senior Vice President of Medical Care at Minnesota Community Care.
Dr. Woods is passionate about addressing social and structural drivers of health, health inequity, and historical trauma. She is driven to provide exceptional care for CHSI patients.

Medical Providers
Kate Suda is a Physician Assistant who has provided care from CHSI's Grafton clinic since 2023. A native of the Red River Valley, Kate earned her bachelor's in psychology from NDSU and her M.S. in physician assistant studies from the University of North Dakota. Kate is licensed in Minnesota and North Dakota, and she is certified through the National Commission on Certification of Physician Assistants. Kate lives with her husband Derrick and their two sons in Grafton. In her free time, she enjoys cooking, baking, and gardening.

Aundria Eoff is a board-certified Family Medicine Physician who practices at our Rochester clinic. Aundria earned her bachelor's in biomedical engineering from the University of Arkansas. She attended the University of Missouri School of Medicine for medical school and completed her Family Medicine Residency training at the University of Kansas Medical Center. She stayed and did full scope family medicine at the University of Kansas for two years. She moved to Rochester, Minnesota with her husband and two dogs, and they welcomed a son shortly after relocating to Minnesota. Aundria's clinical focus is to make guideline-driven care attainable to all patients in the community. She loves the large scope family medicine entails, and especially enjoys chronic disease management, women's health, pediatrics, and prenatal care.

Stacy Olson is a Psychiatric Mental Health Nurse Practitioner who practices at the Willmar, MN clinic at CHSI. She earned her bachelor’s degree in nursing from Saint Cloud State University and her Nurse Practitioner degree from Walden University. Stacy’s professional background includes more than ten years of nursing experience, including inpatient psychiatric care. She is trained in cognitive behavioral therapy (CBT), motivational interviewing, dialectical behavioral therapy (DBT), and Screening, Brief Intervention, and Referral to Treatment (SBIRT).
Stacy’s clinical interests include providing care for patients with depression, bipolar disorder, anxiety, PTSD, and substance use disorders. Stacy works with children aged six years and older, adolescents, adults, and older adults. She is excited to orchestrate the best individualized mental health treatment plan for her patients to support them with their recovery goals.

Lucy Brink is a Nurse Practitioner who practices from our Willmar clinic. Lucy earned her bachelor's in nursing from the College of St. Benedict and her M.S. from St. Catherine University. She has worked as a nurse practitioner for 10 years, including six years at a diabetes center, and she is board certified as an adult/gerontological NP and a diabetes care and education specialist. Lucy lives in Willmar with her husband Dan and their two children. She enjoys spending time outdoors, traveling, cooking, and learning.

Marie Anderson is a Nurse Practitioner in our Grafton clinic. She joined CHSI in 2023 after serving many years in family practice. Marie earned her bachelor's in nursing from the University of Mary and her M.S.N. from the University of North Dakota. She is licensed in both North Dakota and Minnesota and certified through the American Nurses Credentialing Center. In her free time, Marie enjoys gardening, biking, hiking, and reading. She lives in Drayton with her husband, and together they have three children and 6 grandchildren.

William "Patricio" Aleman is a Physician Assistant providing care in our Rochester clinic. Patricio earned his M.D. from Central University of Ecuador, a master's in physician assistant studies from the University of California - Davis, and a master's in adult education from the University of Minnesota - Rochester. He brings decades of experience in primary care at Mayo Clinic and has been certified by the National Commission on Certification of Physician Assistants since 1990. Patricio lives with his wife in the Rochester area and enjoys reading, traveling, and spending time with his children and grandchildren.

Behavioral Health Providers
Gracia Do is the Behavioral Health Supervisor and a Licensed Professional Clinical Counselor at CHSI. She earned her bachelor's degree from Minnesota State University and her M.S. from St. Mary's University. Gracia brings almost a decade of experience providing therapy to individuals of all ages, and she is trained in eye movement desensitization and reprocessing (EMDR) therapy, trauma-focused cognitive behavioral therapy, and dialectical behavioral therapy. Her expertise includes the treatment of post-traumatic stress, depression, anxiety, panic attacks, and relationship conflicts. Gracia enjoys travel, interior design, working out, and spending time with family and friends.

Dental Providers
Dr. Philip Sallberg serves as the lead dentist and dental director at Community Health Service Inc. He graduated with his D.D.S. from the University of Minnesota and spent almost 40 years in private practice in Ada and Roseau, Minnesota. While practicing in Ada, he also worked with Migrant Health Service Inc (CHSI's predecessor) to provide evening dental service. He now provides comprehensive dental care for CHSI's patients. Dr. Sallberg lives with his wife in the Fargo area and spends his time with their children and grandchildren — or playing pickleball!
Dr. Sallberg's favorite part about working at CHSI is helping people and practicing dentistry. He loves being a dentist!
