Appointment Information

All Appointments can be made by calling your local clinic. This includes medical, dental, behavioral health and substance abuse or by calling the toll free number and asking to be transferred to your clinic of choice.

Eligibility

Everyone in the community and surrounding areas are eligible for all CHSI services. If you have worked in the agriculture business within the past 24 months you may qualify for vouchers. A voucher may be used to cover a portion of limited services outside of our facility, such as prenatal care appointments. If you qualify, a voucher can also be used to help cover the cost of insulin. The patient is charged a nominal fee if they accept a voucher.

What to bring to your first appointment:

  • Photo I.D.
  • Insurance card(s)
  • Bottles of current medications, vitamins and supplements
  • Sliding fee scale applicants - Proof of Income

RX Refills

  • Call your pharmacy directly and request a refill, even if your prescription bottle indicates “no refills”. The pharmacy will contact your medical provider for additional refills
  • Allow at least 72 hours for prescription refills. No refills are available after office hours, on weekends or on holidays.
  • It is important to schedule an appointment at least every 6 months with your medical provider when you are taking medications for chronic conditions such as hypertension or diabetes.
  • Please remember Community Health Service Inc. (CHSI) has a policy that prevents your provider from prescribing a controlled substance (scheduled I-IV).
