The Community Health Service Inc. medical team provides quality primary care for everyone. Everyone.
Our four clinics in Rochester, Moorhead, Willmar, and Grafton are family-friendly, and our medical staff take a patient-centered approach to your care, so you are an active participant in your own health plan. The care you receive at CHSI is also informed by the many different aspects of your life: physical, mental, emotional, spiritual, cultural, and social. Our providers, nurses, and support staff take time to understand your goals and work with you on an individualized plan for your well-being.
We have multiple, bilingual staff members and can accommodate all of our patients' language needs with a live, face-to-face video interpreter. Our documents are translated into multiple languages and requests for additional translations can be provided.
CHSI accepts most coverage plans, including private insurance, Medicaid, and Medicare. Patients may also qualify for our sliding fee payment scale and/or discounted prescription drug prices through 340B.
Same-day and walk-in appointments are available.

Dr. Cynthia Woods
Chief Medical Officer
Dr. Cynthia Woods is an internal medicine physician and serves as CHSI's Chief Medical Officer. Dr. Woods earned her doctorate in osteopathy from Philadelphia College of Osteopathic Medicine and a B.S. in biology from Baylor University. She completed her residency at Penn State Hershey Medical Center, and she is certified by the American Board of Internal Medicine. Dr. Woods previously served in leadership at Open Cities Health Center and as a staff physician at Crescent Community Health Center. In addition to her work at CHSI, she currently serves as the Senior Vice President of Medical Care at Minnesota Community Care.
Dr. Woods is passionate about addressing social and structural drivers of health, health inequity, and historical trauma. She is driven to provide exceptional care for CHSI patients.

Kate Suda, PA-C

Kate Suda is a Physician Assistant who has provided care from CHSI's Grafton clinic since 2023. A native of the Red River Valley, Kate earned her bachelor's in psychology from NDSU and her M.S. in physician assistant studies from the University of North Dakota. Kate is licensed in Minnesota and North Dakota, and she is certified through the National Commission on Certification of Physician Assistants. Kate lives with her husband Derrick and their two sons in Grafton. In her free time, she enjoys cooking, baking, and gardening.
Kate values CHSI's commitment to comprehensive care and strives in her own practice to help patients strengthen body and soul.

Lucy Brink, NP
Nurse Practitioner
Willmar Clinic
Lucy Brink is a Nurse Practitioner who practices from our Willmar clinic. Lucy earned her bachelor's in nursing from the College of St. Benedict and her M.S. from St. Catherine University. She has worked as a nurse practitioner for 10 years, including six years at a diabetes center, and she is board certified as an adult/gerontological NP and a diabetes care and education specialist. Lucy lives in Willmar with her husband Dan and their two children. She enjoys spending time outdoors, traveling, cooking, and learning.
Lucy is passionate about chronic disease management and motivated to make an impact in her community.

Marie Anderson, NP

Marie Anderson is a Nurse Practitioner in our Grafton clinic. She joined CHSI in 2023 after serving many years in family practice. Marie earned her bachelor's in nursing from the University of Mary and her M.S.N. from the University of North Dakota. She is licensed in both North Dakota and Minnesota and certified through the American Nurses Credentialing Center. In her free time, Marie enjoys gardening, biking, hiking, and reading. She lives in Drayton with her husband, and together they have three children and 6 grandchildren.

Richard Mokua, NP
Nurse Practitioner
Rochester Clinic
Richard Mokua is a Nurse Practitioner in our Rochester clinic. He joined CHSI in 2022 after specializing many years in nutrition and internal medicine. Richard was born and raised in southwestern Kenya and studied food science and education before migrating to the United States, where he earned a B.S. in nursing from UW-Eau Claire, an M.S. in nutrition from UW-Stout, an M.S. in nursing from Winona State University, and a doctorate in nursing practice from Chamberlain University. Prior to joining CHSI, Richard practiced internal medicine at Mayo Clinic, and he's served in the U.S. Army Reserves since 2009. Richard spends his free time with his family and enjoys jogging and DJing.

W. Patricio Aleman, PA-C

William "Patricio" Aleman is a Physician Assistant providing care in our Rochester clinic. Patricio earned his M.D. from Central University of Ecuador, a master's in physician assistant studies from the University of California - Davis, and a master's in adult education from the University of Minnesota - Rochester. He brings decades of experience in primary care at Mayo Clinic and has been certified by the National Commission on Certification of Physician Assistants since 1990. Patricio lives with his wife in the Rochester area and enjoys reading, traveling, and spending time with his children and grandchildren.

Aimee earned a B.S. in elementary education from Columbia International University and her A.D.N. from Rochester Community and Technical College. She's currently pursuing a master's degree in transcultural nursing from Augsburg University. Aimee has performed in a variety of roles at CHSI since she joined the organization in 2010 and now leads many of our efforts toward clinical excellence. Aimee enjoys climbing, reading, and spending time with her four children and her dog. She's also becoming a plant mom.

Alma Gomez
Interpreter
Rochester Clinic
Alma Gomez is a Spanish-language Interpreter who works from our Rochester clinic. She first worked as an executive assistant in Guadalajara, Mexico, and then earned her GED and CNA after she migrated to the United States. She holds several certifications, including one in MNSure Navigation. Alma lives in St. Charles, MN, with her husband and one of their four sons. In her free time, she leads several programs with her church, and she enjoys walking, dancing, and singing.

Claudia Cazenave, RN

Claudia Cazenave is a Registered Nurse who works in our Rochester clinic. Claudia earned her associate degree in nursing from Rochester Community and Technical College and subsequently passed the NCLEX to become a licensed RN. She also has an associate degree in business administration from the Propam School of Business in Santiago, Chile. Claudia is bilingual, speaking English and Spanish, and she is a certified diabetes care and education specialist. In her free time, she enjoys travel, hiking, kayaking, and painting. She lives in the Rochester area with her husband. They have one child.

She brings several years of experience in healthcare and customer service to our operations, having previously worked in security and front desk roles in clinical and family practice settings. Hope Barry is a Registered Nurse and our Care & Referral Coordinator based in our Grafton clinic. Hope brings more than 35 years of experience in healthcare, progressing from CNA, CMA, LPN, and RN to her present position. She is licensed in Minnesota and North Dakota, certified as a diabetic educator, and a member of the Care Coordinator Association of Minnesota. Hope lives in rural Larimore with her husband, and in her free time she enjoys reading, being outdoors, and spending time with her friends and family.
Hope appreciates that she is heard as a CHSI employee and values the opportunity to support many patients who might otherwise have little to no health care access.

Jessica Mendoza is a Patient Relations Representative in our Moorhead clinic. She brings quality people skills to our patient registration and intake process. After high school, Jessica joined the Army, where she continues to serve as a wheeled vehicle mechanic with the 141st Maneuver Enhancement Brigade. In her free time, Jessica enjoys listening to music, painting, drawing, and skateboarding. On Sundays, she plays guitar for her church.

Olga Mendoza is a Medical Assistant who works in our Rochester clinic. She joined CHSI in 2014 as a Patient Relations Representative and was then promoted to Office Manager. She recently started the MA apprenticeship program and will soon complete her certification. Olga has two sons and lives in the Rochester area. In her free time, she enjoys soccer, volleyball, hiking, and Marvel movies.

Renae graduated from Northland Community and Technical College in East Grand Forks with an AAS in Practical Nursing, and she is currently enrolled at Chamberlain University in pursuit of her B.S.N. She brings extensive experience in long-term care and ambulatory clinic operations, and she is licensed in both Minnesota and North Dakota. Renae lives in Drayton with her husband Justin and their dog and cat. She enjoys traveling, gardening, and spending time with family and friends.

Terra is a nationally certified medical assistant and holds years of experience from a variety of healthcare practices, including internal medicine, ophthalmology, occupational health, and gynecology and obstetrics. She enjoys spending her free time outside, especially swimming and camping. She lives in Fargo with her husband and four children.
